Residential options offer community-based housing that can include supported living services or independent living options. The main distinction for residential housing is that individuals live within a home owned by an agency.
Nashville, TN - Our Place Nashville partners with non-profit organizations to offer housing called Friendship Houses for adults with developmental disabilities. They providing community based housing where graduate students and adults with developmental disabilities can live together and support each other.

Home Care
Home care services can include personal assistance, supportive living, and/or medical assistance services. The main distinction is that these services are provided within an individual's own home.
Office location: Brentwood, TN - Easterseals Tennessee provides personal assistance, supported living, and supported employment programs throughout middle and west Tennessee for persons with disabilities.
Winchester, TN - New Life TN offers a Personal Assistance Program that supports people who need assistance on a day-to-day basis with tasks such as cleaning, grocery shopping, and leisure activities.
Locations: Cookeville; Livingston; McMinnville; Lafayette; Sparta, TN - Pacesetters, Inc. provide individuals with disabilities a variety of residential supports including Residential Habilitation (agency owned homes with 2-3 people); Supported Living (consumer homes/apartments with 1-3 people); Semi-Independent Living (individual homes/apartments with intermittent supports); Family Model (non-family homes with continuous supports for 1-2 people); and Respite (relief services in family or agency homes – typically for people already receiving our day supports).

Family Model
The Family Model is a community-based residential alternative for adults with disabilities that encompasses a variety of support options for individuals living in a home with trained caregivers (other than the individual's own family) in an adult foster care type of arrangement.
Smyrna, TN - Tennessee Family Solutions offers a range of services for persons with disabilities including the following: ICF/IID, Medical Residential, Respite, Supported Living, and Family Model.
